makes changes both features and styles ...

makes many changes in
1. remove logon logoff checkbox. they aren't needed anymore because of algorithm changes.
2. add excel output button
3. change database format due to server format change
4. adds footer
5. make few design ajustments, including text field shadow , etc...

add placeholder and width of time field
